Below are forms which you can access to help us with your ministry. Select the form you require and follow the steps. 

Our Finance Committee will receive the forms by email and will process each one according to the guidelines we have set in place.

Expense Reimbursement (ER)

Expense Reimbursements (ER) may be used for ministry expense which we need to reimburse you for. Please click the link below, enter all relevant information and submit.

Work Order (WO)

Work Order (WO) forms are in place to help keep LifeChurch aware of needed repairs, maintenance and upgrades to its campus. Use this form to submit any known issues and the appropriate team member will be made aware of the Work Order.

Purchase Order (PO)

Purchase Order (PO) forms are designed to empower our ministries at LifeChurch with needed one time or ongoing budget increases to help progress the mission. Please use this form as a wise steward of the resources God has given LifeChurch to utilize.
