Incident Report (IR)

Incident Report (IR) forms are in place to help LifeChurch leadership stay informed of any safety, behavioral, medical, or facility-related incidents that occur on campus. Submit this form as soon as possible following an incident so that leadership can respond appropriately, preserve any evidence, and maintain proper records.
